Required Eligibility Documentation

 

All applicants are required to register on OhioMeansJobs.com prior to the intake appointment
 
Applicants under the age of 25 must also complete the WorkKeys assessments on OhioMeansJobs.com
 
This information is necessary for determining your eligibility for the program. You must bring all applicable documentation on the day of your intake appointment.
 
1.) Valid Driver's License or another valid government issued photo ID.
 
2.) Social Security Card or certified printout of social security number from your local social security office. You can request a replacement card at http://www.ssa.gov/
 
3.) Birth Certificate (US born applicants only); Citizenship/Alien Status Registration Card.
 
4.) Proof of Residency - Document containing your name and current address that has a date within 30 days of your intake appointment. (Utility bills, rent receipt, postmarked letters) P.O. Boxes are not acceptable.
 
5.) Income Verification - We will need to verify the gross household income and/or means of support for the previous six (6) months. This includes income for you and your spouse. If you are under 24 and are supported solely by parents, we will need their income.
 
   Wages - If you or another household member has worked within the past six (6) months, you will need to provide the most recent paystub (sl for each employer as well as a paystub from six months ago. These paystubs must indicate gross year to date income. If this 6-month period falls within 2 separate calendar years (ex. November 2013-May 2014) you must provide a copy of the W-2 tax form for each applicable employer as well.
 
   Other income types that require documentation include, but are not limited to

• VA Benefits
• Retirement/Pension
• Social Security Benefits
• Disability Income
• Soldiers Relief
• Sub Pay
• Child Support/Alimony
• Cash Assistance/Food Stamp Benefits
• Active Duty/Reservist Pay
• Insurance/Annuity Payments
 
6.) Dislocated Workers - If you have ever filed for unemployment benefits, we will need your Determination of Eligibility letter. This letter is provided to you when your claim is initially approved or denied. Even if you are denied for having insufficient earnings or because you worked for an employer not covered under state unemployment compensation law, you may still qualify as a dislocated worker. Knowing your log-in information for your state's unemployment website will usually give access to these documents. Other documentation we would need: • Unemployment benefit statement showing remaining benefits (most recent stub in Ohio) • Layoff Notice (If there is no unemployment) • Warn Notice (If applicable) • Trade Documentation (TRA/TAA) (If applicable) Displaced Homemaker - Another way you can possibly be classified as a dislocated worker is if you were dependent on the income of a spouse and no longer receive that income due to divorce or death. To determine this, we would need to see a copy of the divorce decree or death certificate.
 
7.) Veterans - a copy of your DD form 214 (undeleted) listing the type of discharge. You can register at https://www.ebenefits.va.gov/ and print the form out.
 
8.) Selective Service - All males born after January 1, 1960, must be registered with the selective service. No proof is needed.
